Registration information may be completed online.
However, you will still need to complete and provide an
Immunization Record
and a
Liability Waiver.
Your email address will be your user ID and you will need to select a password.
We do not share any information you provide.
This process takes several minutes to complete.
You will be asked for basic information about yourself and your dog
followed by a questionnaire about your dog and its environment.
NOTE:
If you do not complete the entire proces, your registration will not be retained.
At the end of the process you will receive a message that your registration is complete.
